Care Coordination

Care Coordination is ideal for children whose families need assistance in coordinating services that they are receiving from several agencies. Care Coordinators maintain consistent contact with families while executing and implementing a child-centered service plan for the family and servicing agencies.  Care Coordination is a key component of a family-driven system of care.  

Who we serve: 

  • Children, birth to 18 years, who meet the following criteria: 

  • Have serious emotional and behavioral disturbances and a DSM-IV diagnosis

  • Are currently involved with at least two service providers, such as Department of Children and Families, Juvenile Justice, a mental health treatment provider, Special Education services, etc.

  • Are currently in an out-of-home placement and need service coordination to return home

How the program works: 

  • Referrals are accepted from individuals and agencies with signed consent of the parent or guardian to be referred. 

  • Care Coordinators provide intensive case management services and act as advocates and liaisons to a Community Collaborative on behalf of the youth and his/her family.   

  • Care Coordinators generally work with 8-12 families for up to six months or longer and assist the families with linkages to services.
